How can novel, AI-driven decision support shape future long term care practice?

ONLINE MEETING – March 19th 2025, 15.30-17.00 pm (CET)

Dear Colleagues,

The Co-chairs of the EuGMS SIG on Long Term Care are glad to invite you to the joint meeting to discuss an important new project on potential use of AI in clinical practice. The meeting is open to all members of EuGMS. We seek your advice and involvement.
Background
Care professionals of older persons with complex chronic conditions often lack appropriate decision support. In the EU funded I-CARE4OLD project, a multidisciplinary international team of 14 partner organisations from 8 countries collaborate developed and tested an innovative decision-support platform for healthcare professionals, to enable personalised care. Agenda

1. Introduction to I-CARE4OLD Prof Graziano Onder
The main mission of the I-CARE4OLD project is to develop decision support for better prognostication of health trajectories and treatment impact in older care dependent persons with complex chronic conditions (CCC) using real world high quality data.
Graziano Onder is a geriatrician and professor at the Università Cattolica del Sacro Cuore and the Istituto Superiore di Sanità both in Rome, Italy. He is scientific director of the EuGMS and fellow of European Academy for Medicine of Ageing, and the interRAI collaborative.

2. Showcasing first experiences with a digital decision support platform with the prognoses and treatment estimates
Anna Maria Hiltunen is an Health and social care expert with 20 years of experience. RN, MSc & PhD candidate and manager at Nordic Health Group dedicated to creating impactful, sustainable health and social care solutions through clinical and strategic expertise

3. Specification of profiles for health trajectories
Elizabeth P. Howard is a a professor in the Connell School of Nursing, Boston College, USA, adjunct scientist at the Hinda and Arthur Marcus Institute for Aging Research, Hebrew SeniorLife, fellow in the American Academy of Nursing and a member of interRAI. Her scholarly activities focus on vulnerable, underserved older adults. As a certified acute and geriatric primary care nurse practitioner, Dr. Howard maintains clinical practice in an internal medicine clinic setting.

4. Individualising non-pharmacological interventions, what works for whom?
Hein van Hout is a professor of care for older people at the Amsterdam University medical center. He coordinates the Aging and later Life program at the Amsterdam Public Health Research Institute. He led multiple international studies and is a fellow of the interRAI collaborative. He coordinates the I-CARE4OLD study, together with Graziano Onder.

Speakers and SIG members discuss the scope, proposed methodology and implications of the project.
